Health Screening Form

We ask you to complete a health screening form to make sure that we look after you to the best of our ability. It is your responsibility to keep us informed of your current state of health and any changes to this during the course of your membership with us. This information is confidential to your instructor and the information is not kept on a database. The instructor reserves the right to refuse to allow you to exercise, if he or she believes you could further injure yourself by participating. If in doubt, you should seek the advise of your GP before commencing your activity.
